If you are planning to travel abroad you may require vaccinations for certain destinations, plus routine travel advice.
Please enquire at least 4 – 6 weeks before travelling (in case specialist vaccines are required).
We ask you to complete a questionnaire either available to download here or for collection at our Reception desk

Please return the completed form to Reception. It will be passed to a Practice Nurse to assess if any vaccinations are required. You will then be contacted and, if required, an appointment arranged.
Please note there are a number of vaccines available on the NHS, but some vaccines are not. If you require vaccines that are not covered by the NHS, there will be a charge for the vaccinations. Please ask the nurse about this.
We can also advise on malaria cover; again, this may incur a charge.
